Concierge Home Care

Broad Street is the only service that combines Care Management and Patient Advocacy with Home Care and Home Nursing. This allows us to serve a variety of needs more effectively, by providing the most comprehensive but customized home service solution available.

Through thoughtful preparation and communication, we work with you to put a plan in place and then adjust and adapt to changing needs. Our expertise in catering to complex, unique, and widely varied healthcare needs, making us well positioned to deliver the very best outcomes.

How Our Home Care Services Begin

Initial Consultation via Phone Call

Broad Street invites you to speak with one of our experienced Nurses, who is also Patient Advocate and Care Manager to understand how our services can align with your unique requirements.

For Families, Partners, and Professionals

We welcome conversations with:

  • Families seeking private nursing or guidance at home

  • Physicians and healthcare professionals

  • Experienced nurses interested in joining a more relationship-based care model

If you choose to proceed:

  • In-Person Assessment is arranged with one of our Patient Advocates and Care Managers at a time that is convenient for you, so we can start working through the details of what services are needed and how they would be delivered (the Plan).

  • Engagement of our Human Resource Department to build a team around you and ensure we put the right resources in place. Identifying the Care Team is an extremely important stage in getting the right Care Solution in place.  In order to secure and allocate the best possible resources, we spend significant time and effort approaching and preparing qualified candidates. This is an investment in your care. This may include a Care Professional(s) or Private Nurse(s), or both, working collaboratively with a Nurse Care Manager.

Once we have a Care Team in place:

  • Care Manager will introduce your Care Team and begin the on-boarding Process, which includes:

    • Development of a Care Plan in collaboration with you, and potentially your Healthcare Network (Physicians, Therapist, or other Clinicians).

    • Walk through the Care Plan with your Care Team to ensure understanding, along with providing any direct instruction.

    • Follow-up Communication and engagement to make any adjustment to the Care Plan

    • The goal of the on-boarding process is to ensure that everyone is on the same page and that the relationships between you and your Care Team is working successfully.

Ongoing Proactive Care Management

  • A Care Manager remains proactively involved in your care, adjusting and adapting to your changing needs and incorporating additional support if or when required.

Patient Advocacy

  • For clients who want a more complete view of healthcare options and services, Broad Street can provide Patient Advocacy services, including interacting with, and helping you navigate your healthcare network.

  • This can include other complementary resources, through our Professional Network. By working with other Industry Professionals, we can expand the solution beyond our focused home care solutions.
