Our Nurse Case Managers don’t just manage medical records and referrals; they actively collaborate with physicians, therapists, and home care professionals to create a seamless care experience.

Our Team
Our experienced Nurse Case Managers and dedicated support staff are at the heart of our services. Acting as extensions of our clients’ healthcare networks, we bring expertise and compassion to every interaction, advocating for optimal outcomes and quality of life. In addition to our dedicated advocacy staff, we have access hundreds of other caregivers, private nurses, and other healthcare resources on the Broad Street team.
Amy Bishop, BSN, RN
Amy is a committed Patient Advocate and Nurse Case Manager at Broad Street Advocates. With 27 years nursing experience to draw from, Amy specializes in guiding patients and their families through times of challenging transition and the complexities of the healthcare system. Previous time spent advocating for hospice patients, critically injured crime victims, oncology patients and those utilizing behavioral health services, Amy is able to bring a unique perspective to the table when fostering collaboration and navigating obstacles that may arise. Amy holds a BSN from Benedictine University.
Verlena Sexton, BS, AAS, RN
Verlena is a dedicated Patient Advocate and Nurse Case Manager at Broad Street Advocates, bringing extensive experience in emergency care, case management, and home-based nursing. She has served as an RN Case Manager for VITAS and the Jessie Brown V.A., providing comprehensive care coordination for veterans and patients with complex needs. Verlena also has a background in emergency nursing with Windy City EMS, delivering critical care at major sporting events and concerts. She holds a BS in Health Care Administration from Southern Illinois University and an AAS in Nursing from William Rainey Harper College, using her expertise to ensure clients receive compassionate, high-quality care.
Amy Provenzano, BS, AS, RN
Amy is a dedicated Patient Advocate and Nurse Case Manager at Broad Street Advocates, bringing over a decade of nursing experience across acute care, medical management, and urgent care settings. She has worked as a Medical Management Nurse at Blue Cross Blue Shield and as an RN in both urgent care and hospital settings, specializing in patient-centered care and healthcare coordination. Amy holds a BS in Nursing from Purdue University and an AS in Nursing from Waubonsee Community College, using her expertise to support clients in navigating complex health challenges with compassion and personalized care.
Valerie Hatfield, BS, BSN, RN
Valerie is a dedicated Patient Advocate and Nurse Case Manager at Broad Street Advocates, bringing 18 years of experience in palliative care, hospice, and acute care nursing. Previously a Hospice Nurse Liaison, she specialized in care coordination, patient advocacy, and guiding families through complex healthcare decisions. Her background as a Clinical Resource Nurse and Charge Nurse further strengthened her leadership and interdisciplinary collaboration skills. Valerie holds a BSN from West Suburban College of Nursing and a BS in Health Studies from Eastern Illinois University, using her expertise to ensure clients receive compassionate, high-quality care.
Grace Meitzler, BSN, RN
Grace is a dedicated Patient Advocate and Nurse Case Manager at Broad Street Advocates, bringing experience in emergency nursing, home infusion, and private duty care. She has also served as a night charge nurse in a private duty setting, where she provided leadership and oversight for complex care needs. Grace holds a Bachelor of Science in Nursing and Spanish from Olivet Nazarene University, and she uses her clinical expertise and bilingual skills to ensure compassionate, personalized support for every client.
Melissa Casadei, BA, RN, CCM
Melissa is a dedicated Patient Advocate and Nurse Case Manager at Broad Street Home Care, bringing over 16 years of experience in nursing, case management, hospice, and home health care. A Registered Nurse (RN) and Certified Case Manager (CCM), she has served as a Clinical Case Manager for Paradigm and Orchard Medical Consulting, specializing in catastrophic injury management and care coordination. With leadership roles in hospice and home health, she has a deep understanding of patient advocacy and interdisciplinary collaboration. Melissa holds a BA from the University of Iowa, using her expertise to provide personalized, high-quality support for clients and their families.
Leslie Baltazar, BA
Leslie is a dedicated Care Advocate at Broad Street Home Care, bringing over 15 years of experience in client services, mental health counseling, and care coordination. Before joining Broad Street, she served as Director of Client Services at Homewatch CareGivers, where she specialized in client advocacy and care planning. With a background as a Mental Health Counselor at Swedish Covenant Hospital, Leslie has a deep understanding of emotional well-being and patient support. She holds a BA in Psychology from Loyola University Chicago and is committed to ensuring clients receive compassionate, personalized care.
Radhika Patel, BS
Radhika is a committed Care Advocate at Broad Street Home Care, bringing experience in behavioral health intake, client relations, and healthcare coordination. She has served as an Intake Supervisor and Insurance Verifier, streamlining onboarding processes and improving client access across multidisciplinary teams. Fluent in Gujarati and Hindi, Radhika brings strong communication skills and a deep understanding of culturally responsive care. She holds a Bachelor of Science in Psychology from Drake University, using her expertise to support clients with clarity, compassion, and efficiency.
