At Broad Street Advocates & Private Nursing, we talk a lot about the Continuum of Care - but what does that really mean?

In the traditional healthcare world, "continuum of care" usually refers to a sequence of medical services: hospitalization, rehab, home health. Each stage is distinct, often disconnected, and almost always complicated by insurance rules, clinical thresholds, or system limitations.

We see it differently.

At Broad Street, the Continuum of Care is a relationship-driven, personalized journey - not a series of handoffs. It’s a living, evolving model that adapts to what each client needs, when they need it, delivered by people who know them well.

A More Human Approach to Care

We believe that care should be continuous, not fragmented. That support should serve the person, not the protocol. And that the best outcomes come from knowing someone deeply, reading beyond their chart.

That’s why we built our model to incorporate multiple services under one roof, all managed by a consistent team:

  • Advocacy - Navigating the system, coordinating care, and representing the client’s best interests

  • Private Nursing - Nurses who act as caregivers, offering both clinical support and everyday assistance

  • Home Care - Trained caregivers who provide non-clinical help with daily activities

  • Wellness Concierge - Wellness planning, health monitoring, and preventative strategies

  • Discharge Planning  - Helping clients move safely and confidently from hospital or rehab back home

Each of these services can stand alone, but they work best together. That’s the continuum.

  • A client starts with Advocacy and Concierge Wellness support, creating a personalized health plan, reviewing medications, attending appointments, and setting up systems for long-term well-being

  • Over time, as health conditions become more complex, they transition to Private Nursing, allowing a Nurse who already knows them to provide more advanced care, monitor for changes, and ensure clinical oversight

  • As daily needs begin to increase, they add Home Care, bringing in a consistent caregiver to help with meals, errands, and personal care, all coordinated by the same Advocate

  • Throughout it all, the same Broad Street team stays engaged, adapting the care plan as needed, communicating with family and physicians, and offering continuity that reduces stress and prevents gaps in care
